Yuzuru Otonashi is the main protagonist of the anime Angel Beats!. He is an amnesiac high school student who wakes up in an unknown school, only remembering his family name, and discovers that he is in a place called "Afterlife" when he gets killed and wakes up hours later with no injuries to his body. Otonashi then joins SSS, a rebellious group led by Yuri Nakamura which is dedicated to stopping the mysterious Angel, whom they believe to be associated with God, their sworn enemy. Despite the group's objections, he gets personally involved with the eponymous student and eventually develops feelings for her.

Yuzuru's backstory shows that he went through an "emo" phase as a teenager, desperately searching for a reason to live. His only known relative is younger sister Hatsune, who suffered from a terminal illness. After his sister died, he decides to enroll in a university to study for a medical course. But on the day of his entrance exam, the train he was riding crashed inside a tunnel, trapping all survivors. He strikes a friendship with fellow stranded passenger Igarashi, and together they helped the other survivors while they were awaiting rescue, with Otonashi using his medical knowledge. Knowing that death is a real possibility, Otonashi signs his life insurance card to donate his organs so he can help others beyond death, prompting others to do the same. He dies just as rescuers have arrived to open the tunnel.
In the final episode, it is revealed that Otonashi's beating heart was successfully transplanted to Angel when she was undergoing an operation in the past, hence the title. He confesses his feelings to Angel, but she fulfills her goal in the afterlife by giving her thanks to the one that saved her life and then disappears, leaving him behind.
The anime was co-created by Key and Dengeki G's Magazine, produced by animation studio P.A. Works, and was originally broadcast in 2010. Otonashi is a strong competitor in the male division of International Saimoe League due to Angel's influence, and has won 2 pendants and appeared in the semi-finals twice (2016 and 2017). He is also one of several males to have appeared in every ISML edition from 2011-2013 male exhibitions to the present - with the only exception being the 2014 trap exhibitions.
